I will go with A. I can't see any harm in this but I'm not a doctor.

The correct answer to question # 19,

is B for NCAA; and A for HS.

RULE 1; The Game, Field, Players and Equipment.

SECTION 4. Players and Playing Equipment.
            Optional Equipment.
            ARTICLE 6.
            c.
Eye shields. Eye shields must be clear, not tinted, and made from molded or rigid material. Eyeglasses and goggles also must be clear and not tinted. No medical exceptions are allowed.

or does the rule only cover glasses and goggles put directly on the face, and not helmets?

I would say that is a direct violation of the rule. Wow, that looks like blinders.
